New Patient Registration
Please ask the reception whether you live in our Catchment area Chingford E4. If you live within our catchment area ,you are welcome to register.Eligibility can be confirmed from your your address.We will not discriminate you on the grounds of race ,religion, gender, class,age, medical condition, disability and sexual orientation.
How to register:you can register online or bring the form below.To register please bring the following:
Photographic ID,Passport,and one proof of address.It can be bank statement,utility bill(gas, electricity,or water)council bill or landline telephone bill.Unfortunately we do not accept mobile phone bills.If you were not registered in the UK you can bring EU card or home office papers.

Our policy is that all patients registering with the practice must attend a routine medical check up with the practice and if taking medicine or undergoing any treatment must see the doctor as well. These checks should be done within three months of registering the practice. You may not be registered at the practice if you do not attend this health check.
We expect all patients undergoing treatment to have regular checks thereafter.

A new patient Health check for 0-5 years old: This form cannot be submitted electronically online.Please download the form and complete for all under 5's. kindly remember to bring your child's Red book and details of immunisations when attending the surgery.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
